HCBS Waiver Programs

Next Steps

Learn more and apply

You can find out more about Iowa’s waiver programs on the Health and Human Services (HHS) website.

To apply for a waiver when you already have Medicaid, go to your local Health and Human Services (HHS) office and say that you want to request waiver services. They’ll have you fill out a one-page Home and Community-Based Services (HCBS) Waiver Request.

To apply for a waiver when you don’t have Medicaid, fill out a Medicaid application and be sure to select your preferred waiver. You can apply:

Note: If you use a paper application, make sure to fill out Appendix A and select which waiver you want to apply for.
